Field Trip to Six Flags will be on Wednesday, May 28
A Field Trip Authorization, Emergency Medical Information form and Authorization and Liability Waiver forms are attached. The donation cost for the trip is $135.00 which covers bus transportation, lunch (provided by the park), and admission to the park. If you require a vegetarian meal please write it on the Field Trip form. We will send out more information prior to the event.
This donation is non-refundable due to the prepayment requirements for use of park facilities and buses. Donations can be made either through Future Fund or check to Hart Middle School. CASH will not be accepted. All forms with donation receipt or check should be turned into the office by the March 27 deadline. Due to the prepayment requirement of the bus company and Six Flags, NO payment or forms will be accepted after the March 27 deadline. All checks/money orders should be made payable to Hart Middle School. If we do not receive enough donations to pay for the trip we will have to cancel.
Please be sure that your student returns the following items on or before Thursday, March 27.
- Field Trip Authorization Form with parent/guardian signature.
- Emergency Medical Form with parent/guardian signature.
- Authorization and Liability Waiver Forms with initials and signatures.
- A check/money order or credit/debit card (cash will not be accepted) can be used for your donation in the amount of $135.00 (Please make check/money order payable to Hart Middle School and write your child’s full name and ID # in the lower left hand corner of the check). Completed forms & donations must be returned to Mrs. Garrett in the front office by Thursday, March 27.
STUDENTS WHO DO NOT RETURN THE REQUIRED PAPERWORK BEFORE THE DEADLINE WILL NOT BE ABLE TO ATTEND. DUE TO THE PREPAYMENT REQUIREMENT BY THE BUS COMPANY AND SIX FLAGS NO PAYMENTS OR FORMS WILL BE ACCEPTED AFTER THE MARCH 27 DEADLINE.
On the day of the trip, students are to arrive at school at 8:45 a.m. and report to their 1st period class. (Be on time!) Buses will leave Hart promptly at 9 a.m. Buses will arrive back at school between 4:00 and 4:15 p.m. All students must ride the bus to and from Six Flags. Students must provide their own transportation home from school after the return to Hart. Hart staff and cleared adult volunteers will be chaperoning the trip. All school rules will apply while at the park. Non-compliance could result in non-participation of students in the promotion ceremony on Thursday, May 29.
Students who choose not to participate, or who are not eligible to participate in the trip, will be required to attend school at the regular time 9:36 am-3:00 pm.
